• assessLog() and formatLog() judge each deviation by the state of its loop: unadjudicated, no action, action open, rejected or applied.
• An adjudicated deviation with no applied action is a failure, not a handled entry.
• repeatOffenders() finds steps deviated from by several distinct operators. Those are reported as a critical procedure finding, above the individual entries, even when every incident was closed.
• An empty log is reported as not assessed.
